Request for Mask Accommodation on Campus - University of Toronto, St. George Campus

Students who identify as having a medical or disability-related barrier to wearing a mask while on campus, or who have in specific circumstances where health and safety is at risk been asked by a manager or academic administrator to provide supporting documentation to the University, are being asked to complete the questionnaire below to facilitate a consultation with an accessibility advisor to determine eligibility for a mask accommodation.    Completion time is approximately 5 to 7 minutes. 

Students may be asked to: 

•  Provide documentation about their medical or disability-related barrier(s) 
•  Obtain additional documentation following the consultation, where warranted. 
•  Register formally with Accessibility Services, if there is an identified need.
